Priority skills for equity-focused, evidence-based cancer control in community-based organizations: A group concept mapping analysis with academics and practitioners

Introduction:
Community-based organizations (CBOs) are important equity-promoting delivery channels for evidence-based interventions (EBIs). However, CBO practitioners often cannot access needed support to build EBI skills. Additionally, the capacity-building literature is hindered by inconsistent definitions, limited use of validated measures, and an emphasis on the perspectives of EBI developers versus implementers. To address these gaps, we explored commonalities and differences between CBO practitioners and academics in conceptualizing and prioritizing core EBI skills.
Methods:We utilized Group Concept Mapping, a mixed-methods approach connecting qualitative data (e.g., regarding the range of critical EBI skills) and quantitative data (e.g., sorting and ranking data regarding unique skills) to create conceptual maps integrating perspectives from diverse participants. A total of 34 practitioners and 30 academics working with cancer inequities participated in the study.
Results:Participants nominated 581 core skills for EBI use, and our team (including practitioners and academics) identified 98 unique skills from this list. Participants sorted them into conceptual groups, yielding five clusters: (1) using data and evaluation, (2) selecting and adapting EBIs, (3) connecting with community members, (4) building diverse and equitable partnerships, and (5) managing EBI implementation. The ordering of importance and presence of skill clusters were similar across groups. Overall, importance was rated higher than presence, suggesting capacity gaps.
Conclusions:There are helpful commonalities between practitioners’ and academics’ views of core EBI skills in CBOs and apparent capacity gaps. However, underlying patterns suggest that differences between the groups’ perceptions warrant further exploration.
Enabling community input to improve equity in and access to translational research: The Community Coalition for Equity in Research

Background:
The COVID vaccine trials illustrated the critical need for the development of mechanisms to serve as a bridge between least advantaged communities and researchers. Such mechanisms would increase the number of studies that are designed with community needs and interests in mind, in ways that will close gaps rather than widen them. This paper reports on the creation of the Community Coalition for Equity in Research, a community-driven resource designed to build community capacity to provide researchers with credible and actionable input on study design and implementation and increase researchers’ understanding of factors that influence community support of research.
Methods and Results:We provide a description of the Coalition’s structure and process and an evaluation of its first year of operation. Researchers rated their experience very positively and reported that the Coalition’s review will improve their research. Coalition members reported high levels of satisfaction with their participation and the processes set up for them to engage with researchers. Members also largely agreed that their participation has value for their community, and that it has increased their interest in research and the likelihood that they would recommend research participation to others.
Conclusions:The Coalition represents a model for increasing two-way engagement between researchers and the larger community. We are optimistic that the Coalition will continue to develop and grow into a vibrant entity that will bring value to both investigators and our local communities and will increase the consideration of equity as a foundational principle in all translational research.
Associations between untargeted plasma metabolomic signatures and gut microbiota composition in the Milieu Intérieur population of healthy adults

Host–microbial co-metabolism products are being increasingly recognised to play important roles in physiological processes. However, studies undertaking a comprehensive approach to consider host–microbial metabolic relationships remain scarce. Metabolomic analysis yielding detailed information regarding metabolites found in a given biological compartment holds promise for such an approach. This work aimed to explore the associations between host plasma metabolomic signatures and gut microbiota composition in healthy adults of the Milieu Intérieur study. For 846 subjects, gut microbiota composition was profiled through sequencing of the 16S rRNA gene in stools. Metabolomic signatures were generated through proton NMR analysis of plasma. The associations between metabolomic variables and α- and β-diversity indexes and relative taxa abundances were tested using multi-adjusted partial Spearman correlations, permutational ANOVA and multivariate associations with linear models, respectively. A multiple testing correction was applied (Benjamini–Hochberg, 10 % false discovery rate). Microbial richness was negatively associated with lipid-related signals and positively associated with amino acids, choline, creatinine, glucose and citrate (−0·133 ≤ Spearman’s ρ ≤ 0·126). Specific associations between metabolomic signals and abundances of taxa were detected (twenty-five at the genus level and nineteen at the species level): notably, numerous associations were observed for creatinine (positively associated with eleven species and negatively associated with Faecalibacterium prausnitzii). This large-scale population-based study highlights metabolites associated with gut microbial features and provides new insights into the understanding of complex host–gut microbiota metabolic relationships. In particular, our results support the implication of a ‘gut–kidney axis’. More studies providing a detailed exploration of these complex interactions and their implications for host health are needed.
Cova Eirós: An Integrated Approach to Dating the Earliest Known Cave Art in NW Iberia

At Cova Eirós, we discovered 13 panels with paintings and engravings that stylistically point to the final moments of the Upper Paleolithic. Scanning electron microscopy with energy dispersive X-ray spectroscopy and Fourier transform Raman spectroscopy were used to identify charcoal as black pigment. Although contamination from medieval fires inside the cave complicates the dating of these pictographs, analyses of unpainted rock backgrounds allowed calculation corrections for contaminated samples. We used plasma oxidation and accelerator mass spectrometry (AMS) to directly radiocarbon (14C) date two charcoal paintings—confirming that the images are more than 9000 yr old. As these paintings superimpose engravings, these 14C dates also provide a minimum age for an engraving at Cova Eirós that is stylistically Final Magdalenian/Epipaleolithic. This is the first known evidence of Paleolithic cave art in Galicia of NW Iberia.

Therapeutic misconception (TM)—when clinical research participants fail to adequately grasp the difference between participating in a clinical trial and receiving ordinary clinical care—has long been recognized as a significant problem in consent to clinical trials. We suggest that TM does not primarily reflect inadequate disclosure or participants’ incompetence. Instead, TM arises from divergent primary cognitive frames. The researchers’ frame places the clinical trial in the context of scientific designs for assessing intervention efficacy. In contrast, most participants have a cognitive frame that is personal and focused primarily on their medical problems. To illustrate this, we draw on interview material from both clinical researchers and participants in clinical trials. We suggest that reducing TM requires encouraging subjects to adjust their frame, not just add information to their existing frame. What is necessary is a scientific reframing of participation in a clinical trial.

The aims of this Working Group are:
• To ensure that scientific symposia on planetary nebulae take place regularly, ideally every 5 years. These symposia would preferably be sponsored by the IAU;
• To organize and coordinate the Joint Discussions on the subject at the IAU General Assemblies. These discussions should address topics of interest not only to our Division VI but to other Divisions as well; and
• To maintain a Web page with general information about the WG, the activities related to planetary nebulae, and the future meetings and symposia.

We performed a cost-effectiveness analysis to examine the relative efficacy and costs of percutaneous ultrasonic lithotripsy (PUL), extracorporeal shock-wave lithotripsy (ESWL), and surgery for the treatment of upper urinary tract stones. We developed a Markov model with 35 states, cycles of 3 months, and a time frame of 5 years. Probability estimates were derived from a meta-analysis of the published literature. For stones less than or equal to 2 cm, ESWL is preferred to PUL, since it prevents 2 additional days of morbidity and saves $440. For larger stones, PUL is preferable to ESWL, avoiding 4 more days of morbidity, and saving $722. Both ESWL and PUL were superior to surgery. Sensitivity analysis showed that the results are sensitive to ESWL efficacy rates, the stone recurrence rate, and the hospital component of the ESWL cost. Our analysis suggests that although ESWL is preferable, relatively small changes in the efficacy and cost can shift the preferred strategy; in addition, these findings underscore the need for more reliable data.

ABSTRACT
Objective: The goal of the study was to assess the efficacy of sertraline in the treatment of the night eating syndrome (NES). Method: Thirty-four outpatients diagnosed with NES were randomly assigned to receive either sertraline (n = 17) or placebo (n = 17) in an 8-week, double-blind, flexible-dose (50–200 mg/day) study. We used the mixed effects linear regression model to analyze change in the primary outcome measure, the Clinical Global Impression of Improvement Scale (CGI-I). Secondary outcomes included changes in the Night Eating Symptom Scale (NESS), the number of nocturnal awakenings and ingestions, total daily calorie intake after the evening meal, Clinical Global Impression of Severity Scale (CGI-S), Quality of Life Enjoyment and Satisfaction Scale (Q-LES), and weight. Results: Sertraline was associated with statistically significantly greater improvements than placebo in the CGI-I scale. As determined by a CGI-I score of ≤ 2 (much or very much improved), 12 subjects in the sertraline group (71%) were classified as responders versus only 3 in the placebo group (18%). There were also significant improvements in the NESS, CGI-S scales, Q-LES, frequency of nocturnal ingestions and awakenings, and calorie intake after supper. Overweight and obese subjects in the sertraline group (n = 14) lost a significant amount of weight by week 8 (mean = −2.9, SD = 3.8 kg) compared to those in the placebo arm (n = 13) (mean = −0.3, SD = 2.7 kg). Conclusions: In this 8-week trial, sertraline was effective in the treatment of NES and was well tolerated.

cerebellar hypoplasia is a rare malformation caused by a variety of etiologies. it usually manifests clinically as non-progressive cerebellar ataxia with or without mental retardation. we further characterize a syndrome of autosomal recessive cerebellar hypoplasia in the hutterite population, referred to as dysequilibrium syndrome (des). we reviewed 12 patients (eight females, four males; age range 4 to 33y) with this syndrome. patients were examined and underwent a standard set of investigations to characterize better the clinical features, natural history, and neuroimaging of this syndrome. des is an autosomal recessive disorder with distinct clinical features including global developmental delay, late ambulation (after age 6y), truncal ataxia, and a static clinical course. neuroimaging is characterized by hypoplasia of the inferior portion of the cerebellar hemispheres and vermis, and mild simplification of cortical gyri.

The fluorescence x-ray microprobe beamline at the Advanced Light Source (ALS) provides a technique to study almost any sample with excellent trace element sensitivity, high spatial resolution and minimal sample damage. During the last two years, the focused spot size has been improved from 2 μm × 4 μm to 1.6 μm × 1 μm using a pair of elliptically-bent multilayer mirrors. Two sets of mirrors have been produced for operation at 8.5 keV and 12.5 keV with a bandpass (ΔE/E) of 4 %. The intensity has also increased by a factor of 20 to over 1 × 1010 photons/sec by linearly grading the multilayer coating on the mirrors and by operation of the ALS at 1.9 GeV rather than 1.5 GeV.
A wide variety of samples have been studied. Some of the materials science samples that have been studied include gallium nitride samples, polycrystalline silicon solar cells, the liner material of a rocket motor and the central germanium core of an optical fiber.
